The above (including the bill-to) would seem to cover off what Chris suggested 
that LSMB should desire except only for

        2)  Any price adjustment factors (discounts, etc)

which, for first-iteration, could be communicated if-at-all inside the same 
"comment" as may serve the purpose of "Notes" including "I am not sure what to 
bill here…"

PS the bill-to could also, in the case of a new patient (first billing), allow 
the doctor to communicate the default billing for this new patient e.g.

        bill patient / mastercard

        bill Medicare, 1234567890
