[Gnumed-devel] state of the coding work

I have pushed the current state of coding affairs to
gitorious.org.

This includes full circle support for coding episodes:

- add/remove codes from episode edit area
- add/remove codes from per episode (notelet) SOAP edit area

- display codes in EMR tree episode node tooltip
- display codes in EMR tree episode node synopsis
- display codes with episode row in EMR journal

Next up is health issues which is going to be faster since I
figured out all the nooks and crannies during episode
handling.

The code in the repository should be runnable as is. It
needs a freshly bootstrapped v16 database which is also
included in the git tree.
