Following is a comma-limited set of columns sourced for doctor information in BC.CA:

Msc_Adjust,CurSurname,FirstNames,Name,DegreeYear,UnivComp,FamMed,Spec1a,Spec2a,Spec3a,Spec4a,Spec5a,A1-Title,A1-Dept,A1-Addr1,A1-Addr2,A1-City,A1-Prov,A1-Country,A1-PostalCode,A1-Phone,A1-Fax,A2-Title,A2-Dept,A2-Addr1,A2-Addr2,A2-City,A2-Prov,A2-Country,A2-PostalCode,A2-Phone,A2-Fax,A3-Title,A3-Dept,A3-Addr1,A3-Addr2,A3-City,A3-Prov,A3-Country,A3-PostalCode,A3-Phone,A3-Fax,DegreeCode

We can see that the above is non-normalized, providing per-doctor for
- multiple (up to 5) specialty statuses and
- multiple (up to 3) full "locations" (complete address, phone, fax)

So… should

ref.providers

adopt a similar (non-normalized) structure? Or would we fold it back on itself and instead:

- repeat one provider possibly multiple (up to 3 times) when they have more than one location, making each unique on
external_org_id + external_id_code + Address line 1 (Addr1)

but normalizing-out the qualifications?

- or normalizing out the address and phone channels?
