Since there can only be one status per identity it can
technically be a column of dem.identity directly.
The grander plan might be that being active or not is a
property of the *patient* rather than the person and thus it
might be more appropriate for it to live in a future
create clin.patient (
pk serial primary key,
fk_identity integer foreign key
references dem.identity(pk)
);
