It already exists: Cancel the template selection list and
you'll be presented with a file system browser from which
you can pick the patient photo.
Yes, this is a one-off.
And yes, contrary to image-from-template, GNUmed will import
this file regardless of whether the MD5 sum changed over the
editing phase (when the MD5 sum of an image created from a
template hasn't changed over editing GNUmed asks whether to
still import it).
