Would categories include

        Physiotherapists
        Medical Doctors

and if among Medical Doctors there would be

        Cardiologists
        Family Doctors
        Psychiatrists

would the latter be subcategories (in which case this too should be filterable).

There do exist doctors who have dual certifications or other expertise worth 
being able to track and identify. Would that break this model? Are there 
reasons why providers need to be stored in some other table in GNUmed?
