[Gnumed-devel] factoring out current encounter

In the SOAP notelet plugin we can't get rid of the

        Encounter: [Save]

button just yet because we may want to keep the RFE/AOE
fields above the SOAP fields such that those can be edited
in context.

We do gain screen real estate though by removing the

        encounter type / start / end

line and the

        Encounter: [New]

button, though.
