It is a by-product of the implementation and should only
happen if:

        - the address type declared in the placeholder inside
          the template is missing or invalid

        - there is no address registered for that patient for
          the address type declared in the placeholder inside
          the template

There is a mechanism to avoid repeated selection of the
desired replacement for missing values but said mechanism
has not been applied to addresses yet.
