A variation on alerting the user (to avoid to interrupt them) would be to 
dynamically update a display area to show any matches as the user advances 
through some of the creation fields for example enter and then tab beyond

show matches on lastname and 2 characters

        Smith, Alan Brian [dob 2004.12.07]
        Smith, Allan Charles [dob 1957.06.13]
        Smith, Alister Frances [dob 1984.01.01]
        Smith, Mark Allan [dob 1984.01.01]
        Smith, Robert Altuis [dob 1984.01.01]

which could allow the user to visually observe existing identities and cancel 
the creation (or double-click on a match) without requiring a separate modal 
prompt?
