[Gnumed-devel] praxis definition in GNUmed

I have added selecting a praxis in GNUmed.

This allows all sorts of things to hang off from the praxis,
say clin.encounter.fk_location, or future definitions like
Office Hours or some such. This also now allows for an
office address regardless of the provider to be used in,
say, document templates (the placeholder is not implemented
yet, however).

Each database can only have one praxis to which all branches
must belong. Each praxis can have several sites/branches (in
fact, each branch is but a link to a dem.org_unit).
