[Gnumed-devel] improved form output

I have added much more versatile form output disposition.

When a document has been generated from a template the
attached dialog is shown. This potentially works for several
forms at once if that gets used. The user can now

- preview the generated output
- remove a generated document from further processing

- immediately (one-button)
        - print
        - email
        - fax
        - archive into the patient's documents
        - save to disk
  the generated forms

- or do an aggregate of the above by entering
  values/checking boxes and [OK]ing

- if any of the above succeeds the "Note"
  will be added to the chart along with an
  autogenerated note about what has been
  done to the forms

If the appropriate scripts (gm-fax_doc(.bat) and
gm-mail_doc(.bat)) are not available the appropriate
actions are greyed out.
