Personally, I'd actually prefer two templates in that
situation but that's certainly up to your inclination.

Note that GNUmed will pass over the template up to
three times in order to successively replace placeholders.

So what you can do is to include a $text_snippet$
placeholder and create a text expansion macro
containing xelatex code which itself contains the
placeholder to pull data from the database which
you want to include only once. That way you can
select whether to include the xelatex code which
subsequently expands to the desired optional data.

Hint: you'll have to include an ambiguous keyword
part in the text_snippet in order to force
GNUmed to show a list of text expansions such
that you can select it or not depending on the
current situation.
