[Gnumed-devel] GFR estimation

I have made GNUmed know how to estimate GFR from serum
creatinine based on all 3 (4) commonly used formulae
(Cockcroft-Gault, CKD-EPI, MDRD-4; and Schwartz, in
pediatric patients). That'll help in known a value for
comparison given that different publications will use
any of those methods).

GNUmed will also apply heuristics as to which to show by
default, based on age and projected GFR value (at/below 60).

Tooltips will still show calculations based among all methods
and facilitate identifying the "default" method chosen by
way of the value.
