Here is the process in principle

0.) get the gnumed-client and gnumed-server tarball and unpack it - there is 
batch file for that
1.) backup all *.nsi files and rename them to reflect the new GNUmed version
2.) move out all old *.nsi files and *.exe files
3.) open gnumed_client-config.nsh and change the version string
4.) do the same for the gnumed_server-config.nsh
5.) if there is a major release change the database upgrade versions in the 
nsi file
6.) in case of new files/folders being added by the release add them to the 
install and removal section
7.) make a copy of the make-dists batch file, rename it to reflect the new 
GNUmed version and move the old one out
8.) open and edit make-dists batch file to point to the new GNUmed version 
9.) open and edit the gm-patch-dbconfig batch file, run it on a new release
10.) get setup-client.py from folder misc and copy it to the unpacked client 
directory as setup.py
11.) open setup.py and change the GNUmed version string
12.) do the same for the setup-server.py
13.) go to the gnumed client directory, find the gnumed-from-vcs.conf,open it, 
change workplace to 'GNUmed Default'
14.) in the 7zip folder edit the files to reflect the shortly to be produce exe
15.) copy bootstrap-latest2.bat and upgrade_db2.bat from misc into unpacked 
gnumed server directory (subdir bootstrap)
16.) edit timelinelib/config/path.py

Now run make-dists.bat and see the exe files being created
Test the exe files
Upload them for public consumption
