    doc/ms.ms: Fix style and content nits.
    
    * Drop man page section from repeated man page cross references.
    * Fix font style usage errors.
    * Drop reference to AT&T pic manual "available on the Web as a
      reference"; we provide our own updated pic manual these days.
    * Recast description of .EQ argument to speak of "alignment" rather than
      "adjustment".
    * Recase descriptions of columnation macros to be imperative sentence.
    * Clarify that the gutter argument to .MC is also a minimum width.
    * Identify MINGW register as a GNU extension.
