doc/meref.me.in: Clarify discussion of `++` macro.
More carefully describe application of appendix and page numbers to
customized titles.
Try not to alarm readers by suggesting that physical page resequencing
is the only remedy for collected "indexes" coming at the end of the
document. Depending on the output format, resequencing can occur in the
digital domain, reducing tedium.
Also use special character for basic Latin circumflex accent.
