    [man pages]: Fix content and style nits.
    
    Content:
    * Stop referring to the recognition of `.R1` and `.R2` tokens as "new"
      features of GNU refer; they are thus only in a pedantic sense, having
      by now been around twice as long as troff itself had been when they
      were introduced.
    
    Style:
    * Set file names with mixed variable and literal content per our
      recommendation in groff_man_style(7).
    * Use "file" instead of "filename" as metasyntactic variable name, for
      consistency with our other pages.
