groff_mm(7): Fix some table styles.
Some tables inconsistently and illogically typeset the name of the
parameter they were describing outside the tables they were using to
present the possibilites. Format these more conventionally.
Also eliminate a gratuitous level of inset/indentation, affording more
room for explanations. That done, drop duplicative combination of IP
with RS/RE. Use column headings for these tables. Set table column
headings in bold (or bold-italics).
Also use two empty requests where vertical space is expected.
Also use the new tbl(1) style for format descriptions (capitalized
classifiers, lowercase modifiers).
