doc/ms.ms: Revise.
* Drop cross reference to pdfmark.ms, which is no longer distributed
with groff.
* Clarify delimiter selection provide some background.
* Improve terminological discipline: _interpolating_ a register is one
means of defining it, not "accessing" it. The GNU troff `pnr` request
also accesses registers, but never creates one.
* Bump document date.
* Add copyright notices and rendering guidance comment.
