[Groff] Updated m.tmac patch

This patch actually works. :-) It's a part-way solution to getting -mm 
to play nice with grohtml; specifically, it:

        - eliminates header/footer output

        - enables headings (.H 2 "blah" -> <h3>blah</h3>)

The next step, converting list items properly, is going to be more 
difficult. I think it can be handled in m.tmac though; changing .LB, 
.LI, and .LE to emit HTML tags should suffice.

First things first... try out this patch & see if it works for you.
