Groff needs to know you have a font which includes a Ccaron glyph, so this
works for
me:-
printf '.ft U-TR\n\xc4\x8cSSR' | groff -kT pdf | okular -
The standard TR font does not have this glyph defined, but the alternative U-TR
does.
