The `SC` macro itself appears to be unique to this document in Unix V7,
so I assume we can credit Brian Kernighan and/or Lorinda Cherry.  I
don't see it in other documents with a Kernighan byline, so maybe we can
credit the late Ms. Cherry for it.  It seems to have better ergonomics
for latter-day *roffers who are more used to calling macros that take
arguments than to ones which open a diversion and start gobbling up text
for later regurgitation.

The idea to leverage the groff `pdfbookmark` macro at that point,
rousing the impressive machinery of modern PDF viewers, was all Deri's.
