According to this file, it looks like Smart card reader is connected in port 8 at internal USB2 hub Bus 002 Device 002: ID 8087:0024.
Did you try it again with your patch in usbhub.c applied (grub_uint8_t -> grub_uint32_t)? I think it should work now (change bit of port 8 is located in second byte of Hub and Port Status Change Bitmap DWORD, i.e. in part which was affected by the bug...).
