Follow-up Comment #1, task #11364 (project health):

I propose using the following format :

Patient Information
- ID, DoB ...
- QR Code : I believe we should upgrade from EAN-13 to QR
- Picture

The same concept will apply to the newborn wristband

QR is quite common today and no need for special readers...

Any other alternatives / ideas ?
