[Health-dev] Demo database information

Dear all

I just published the initial Demo database setup information, as well
as the main info that we will have for demos / trainings.

Please take a look at it and let me know if you want to change / add /
remove any of the basic info (before we actually start populating it
). The current demo directory will be updated with the actual info
shown in the GNU Health Wikibook, so, we have time to modify it. Once
we have the main patient info, that information should be static,
since it will be the base to add further data !
