[Health-dev] Demo DB updated - ready for testing

Dear community: 

I have updated the GNUHealth Hospital management demo
databases, including the stable (4.0.4) and development version for
upcoming 4.2 

I have also updated the documentation in WikiBooks on how to download
and install them :

Please download and test the demo DB 41 against the latest default GH
kernel from our default (development) mercurial branch and report any
issues here.
