We need to update the gender identity in patient evaluations and reports,
retrieving it from their demographics (which has been revised in 4.2)

The biological sex can always be found in the patient information, restricted
to the medical context.

The reports will also report the current gender identity, when the information
is known and the person wishes to disclose it, otherwise it will be set to
"unknown" or "non disclosed".
