I have followed the gnu health plugin wiki install, but didn't work.

I think the wiki was not clear, is the plugin(opencv) supposed to be installed on server (GNU Health Server) or clients (every user's client)?

Using the web client, yes, but not with the GTK plugins.
On upcoming 3.2, MyGNUHealth native app starting with  PHR.

The GTK camera plugin is for the GTK client on *NIX systems.
