[Newbie] Forms question - Can't create a new record

Hello all,

I can't create a new record whilst in Forms mode and I know I have 
access to the file because I own it and I have tried to toggle the read 
only mode by M-x forms-toggle-read-only and it still won't give me 
write access.

Any ideas?
