Rocky: a good point.  I guess either Patrick or I could volunteer; he may have 
better background in the intricacies of the BlueBook 
(CD-Extra).

One more point: when I query cdio_get_discmode, it returns 
CDIO_DISC_MODE_CD_DA. Should this actually respond with 
CDIO_DISC_MODE_MIXED (God forbid we add some CDIO_DISC_MODE_CD_EXTRA enum and 
really screw things up...).

Thoughts?

PS: because of a couple of other bugs, I have not tested Patrick's patch yet, 
but will by tomorrow morning.

One other small but I guess important thing. If someone could work up
a small image file that exhibits this bug, I'd like to add that as a
test case. Thanks.
