Eliminate LYRIC_MARKUP and LYRICS_STRING tokens

This reduces the number of parser rules and reduced the possibilities
for weird lexer/parser timing problems.

Two commits:

parser/lexer: eliminate LYRIC_MARKUP as a token separate from MARKUP


parser/lexer: don't require separate LYRICS_STRING different from STRING
