I'm just guessing here, and I'm not a piano player at all, but
wouldn't it be reasonable to view the two pairs of hands as two
different instruments and output parts for each "instrument" in a
separate \score block?

Sure, the page numbers would be off, care would have to be taken
(probably using \break) that page breaks happen in the same places,
and some manual collating would have to be anticipated.
