Hi Michael, Francesco,

The technique of generating A4 and then converting to A5 works OK. I use 
it often (either by printing to PDF from my PDF viewer or with tools 
such as pdfjam). However, I find that often these techniques loose 
hyperlinks (in the TOC, for instance) so I would be interested in a full 
LilyPond solution aswell.
