ChangeLog: 

* cmd_merging.cc, cmd_ws_commit.cc, commands.cc, commands.hh: Get rid of
  an obsolete process function that took a string instead of a command_id.

* commands.cc, commands.hh: Introduce a public make_command_id function
  that, given a string, constructs a command_id.  Needed to make the above
  change easier and cleaner.
