ChangeLog: 

* base.hh: New file incorporating inclusions and declarations that
should be visible everywhere.
* HACKING: Document rule for use of base.hh.
* audit-includes: New script.
* Makefile.am: Add new files in appropriate places.
Call audit-includes from distcheck-hook.
Remove no-longer-necessary -D switches.
* throughout project: Update inclusions.
