ChangeLog: 

* Removed all of the branch_state structure. Instead of tracking that
  separately, I'm using the roster of the parent revision.
* Added code to resolve cross edges with commit only paths. It simply
  interleaves between the two paths, inserting the younger blobs
  first.
* in_branch now is a cvs_branchname, instead of a cvs_blob_index.
* Added an unnamed_branch_counter and correctly number through the
  unnamed branches.
* Removed the map of resolved_tags, instead I'm directly adding the
  tag as soon as I encounter the blob.
