ChangeLog: 

First version of bug fix for ticket:6 (bsddb running out of locker
entries in the lock table). The `get_repository` method now returns a
unique repository handle to each caller, and a unique cache handle
therein. Cache connections are closed together with the repository.

CachedMTN has been changed so it doesn't inherit from MTN anymore, but
proxies to an MTN instance.

Also, the cache module has been simplified.
