Simplify add_prefix_matching_constraint(), fix a couple bugs.

- A full ID would match that ID or that ID + 1 (ie, ...ab would
  also match ...ac)
- A prefix matching /f+/ would make it fall off the beginning of
  the prefix when incrementing it to get an upper bound.
