lsof on /usr right before the umount request:

starting to look more and more like a RedHat bug, although
it's very hard to reproduce on other systems (RedHat included).

Additional Info:

Seems to happen just after quotas are turned off in
rc6.d/S01reboot.  Again, as soon as "passwd files mysql" is
changed to "passwd files" the problem goes away.  I'm
willing to do any testing needed to get this resolved.
