I'm not expert enough with wiki to administrate, but I do have a private wiki
setup that I use as a "lab book". Setting up the math/texcv was a bit tedious
(for me). I didn't encounter the "Failed to parse (unknown error):" error.
However, I can confirm that equations on the FEM-fenics page render nicely on
my private wiki.
I'm using mediawiki 1.17.0 with mysql. If it would be helpful, I can provide a
copy of my LocalSettings.php file.
