Remaining old VCS keyword instances

I've removed just about all of the remaining old version control keywords
such as "Author:", "Adapted-by:", "Modified:", "Date:", "Description:". 
The remaining ones are in the attached list.  I was uncertain how these
should be handled and so will defer to you.
