Details:

I thought it somewhat unconventional to have access to the documentation
somewhere other than under "Help".  Here's a patch that will add
"Documentation" to the "Help" menu with sub-menu "On Disk" and "Online".  The
"On Disk" selection acts the same as focusing documentation under the "Window"
menu.  The "Online" selection takes the user to the online documentation at
the GNU website via a browser...

I also changed around the order of Agora and Octave Forge in the list under
"Help" so that the list "Report Bug"/"Visit Octave Forge"/"Visit Agora" goes
from more closely tied to the core developers to less closely tied to core
developers...

I've noticed that these submenus "stick" from time to time.  It seems almost
similar to the "sticking" I've seen in the command window.  I wonder if these
are the result of the same bug.  That might help locate that long-standing
bug.
