Changing the php.ini fixed the problem.

Since mail To: is empty this indicates the fomd data did not get to the server,
probably you need to look at php.ini and see "allow file uploads".
However, you could also try the latest anglemail release and tell me if you
still have that problem. Somewhere along the way I changed the compose "form"
type to x-urlencoded which php does not consider a "file upload" which may
eliminate the problem you are having.

There are not any messages in the Apache Error Log.
This is all that is in the access log.

In the to field, I have a single email address only. I have the same problem
when I click reply (without changing to field at all).
It install, I ectracted the tgz, then modified the header.inc.php (renamed the
.template). The only changes I had are to the database settings and added a
second domain.

I am not sure what is going on with your install.
For the smtp error i think this is caused by a comma (,) after the username you
are trying to send to.
What errors are generated when you try to use compose? check you logs for
apache or php errors
Cheers
