Summary: fix a few category bugs, add and clarify some warnings

There were (and possibly still are) a few places where categories where used
the old way, that is, the code checked for feed in category, when it should
be feed in category.feeds. Fixed those. Also added feed id printing to the
"feed already in category X, skipping" message and a warning to
FeedCategory.__contains__: I don't *think* there are any places in the code
where we should be checking for direct FeedCategory membership. If I'm wrong,
the warning should be removed.
