So no GNU gv problem.Well, my guess is that something is wrong with the postscript file - I have tested
(1) An older gswin32 (2) My self compiled ghostscript on CentOS 5.11 (3) OS X Yosemite Preview (4) Adobe Distiller And all display the file different, i .e. (1) Header and footer ok (2) Like your screenshot (3) Header is missing (4) Page is totaly blank with gray background, page margins are white.
