Patches #219 - #224

Hi Feico,

I applied patches: #219, #220, #222 and #223. Some of them were a bit
changed in compare to your version. Eg. I added HAVE_SYS_TIME_H to
check for this include.

#221:
Could you please look into win32 directory and make it compatible with it?

#224:
+#    define dump       while(0)
+#    define dprintf    while(0)

Are you sure that syntax is correct?
