D'oh, sorry.  Wrong smsd config file (tried a lot of software before finding gnokii).

No USB or any other related messages appear anywhere in any log file or on the console.

Unfortunately, when it stops working a power cycle of the USB modem is required.  Simply re-running usb_modeswitch doesn't help.

I'm leaning towards a USB issue.  Could be the device itself, but it's brand new and seems to work fine on a PC.
