There has to be a way in either scenario you mentioned to suggest that though
these were the dd, final diagnosis is this. And once final diagnosis is
reached, the dd should not be listed as health issues. Don't you think so?

multiple items placed in a single soAp row, in a single visit (in which case
this information would print as part of printing the narrative, but it would
not print as part of the list of Health Issues.
Or is it a *series* of diagnoses, provisionally
1 (at the first encounter)
2 (at the second encounter)
3 (at the third encounter)
where it is the most recent of the above (#3) that would be contained in an
Assessment row?
