Maybe I am wrong, do you mean the below case?
when patientA see doctors: home -> doctorA -> doctorB -> doctorC -> home
When I am doctorB, If patientA tell me that he first see doctorA, and
doctorA suggest he to see me, then I should fill 'derived from' to
doctorA, If I think patientA should see doctorC, then I should fill
'derive to' to doctorC
So doctorA should not fill 'derived form' and doctorC should not fill
'derived to'
Am I right?
