I just updated speech-dispatcher to 0.12.0-RC3 and found that the pulseaudio driver is losing samples for me, making it largely unusable with orca. It works well if it only needs to play one sample at a time but starts to lose audio if multiple samples are queued. If I revert 2ea44e, then it seems to work fine again. For now I have reverted 2ea44e. It was supposed to provide much less potential for underruns, but if that poses problems to some audio set up, better leave it as it is, and hope that in the long run the pipewire driver will just work.
I'd forgotten about that issue I was having, although I'd reverted that commit on openSUSE. Sorry; I meant to try to debug it more, but I probably got distracted with something else, and I don't understand pulseaudio's code very well. I agree that it might not matter much in the long term; it looks as though pulseaudio is being replaced by pipewire, and I only have a system using pulseaudio because it's an older install and I haven't tried to update it to use pipewire's compatibility package.
